Infectious Diseases Behavioral Health Counselor

GENERAL DESCRIPTION:

Carolina Health Centers, Inc. is looking for a Behavioral Health Consultant (BHC) to grow their Behavioral Health Department and join their primary care team. Carolina Health Centers, Inc. is a decorated patient-centered medical home that serves children and families throughout the Lakelands area. The Infectious Diseases (ID) BHC will serve as an integral member of a multidisciplinary care team. This ID BHC will provide ongoing clinical counseling and assist patients in managing and navigating chronic disease care. The ID BHC will prioritize our patients identified with HIV/AIDS, Hepatitis C, and other related infectious diseases. The ID BHC will be joining our Uptown Family Practice location in Greenwood, SC with travel to other sites, as needed.

The ID BHC will:

Conduct screenings and assessments with our patient population
Provide skilled ongoing therapy for patients across the lifespan, prioritizing our infectious disease patient population
Assist the primary care provider in recognizing, treating, and managing behavioral health issues
Utilize evidence-based interventions
Clarify mental health diagnoses
Collaborate with other medical professionals in a fast-paced work environment
Connect patients with additional resources, as needed
Provide same-day warm handoffs as well as real-time consultation, when possible
Work towards reducing stigma, as well as increasing awareness of mental health and chronic disease concerns through peer education and outreach efforts

DUTIES & RESPONSIBILITIES:

Utilize clinical skills to accurately assess and formulate diagnosis as well as treatment recommendations for patients
Develop specific treatment plan to guide patient’s services
Provide individual or family therapy, as clinically indicated
Respond to the mental health needs of primary care clinic
Communicate effectively with an interdisciplinary care team
Provide treatment utilizing evidenced-based modalities (CBT, DBT, Motivational Interviewing, etc.)
Communicate pertinent patient updates in a clear, concise manner
Collaborate with referring medical provider to support patient’s overall health goals
Coordinate patient care with community partners
Document patient encounters in a timely and efficient manner
Participate in department meetings, as scheduled
Complete administrative tasks in a timely manner

In addition, this position requires:

Education:

-Graduate of an accredited program with a Master’s degree in counseling, social work, or a related field

-Experience counseling youth and adults

Work Experience:

-Experience counseling youth and adults

Licensure and Certification:

-Counselors must be fully licensed in the state of South Carolina to provide counseling (e.g., LISW-CP, LMSW, LPC, LPCS, LPCC, or LMFT)

Skills:

-Demonstrated clinical skill

-Ability to communicate effectively with all levels of patients and staff

-Critical thinking, problem solving, and consensus building skills

-Ability to exercise a high level of confidentiality

-Ability to coordinate, analyze, observe, make decisions, and meet deadlines in a detail-oriented manner

Physical Abilities:

-Required to talk and hear in order to communicate with others

-Often required to sit and use hands and fingers, to handle or feel and to manipulate keys on a keyboard

-Occasionally required to stand, walk, reach with arms and hands, and to stoop, kneel, or crouch

-Visual abilities required by this job include close vision with moderate exposure to a computer screen

Work Environment:

This position involves exposure to patient elements. The noise level in the work environment is moderate. Requirement to out-of town and/or overnight travel is minimal.
